Acknowledgement of Demands
Parents acknowledge that adult level hockey involves increased physical, mental, and emotional demands. Parents understand that the pace, physicality, decision making, and skill level differ significantly from junior hockey.
Eligibility & Agreement
I confirm that:
 The player is at least 13 years old and meets the eligibility requirements set by the
relevant governing body.
 The player, parent/guardian, and coaching staff have discussed the suitability of adult
participation and agree that it is appropriate for the player’s development,
confidence, and safety.
 The player will wear protective equipment that meets current safety standards at all
times.
 The player and parent/guardian understand that, while the club will take all
reasonable steps to ensure safety, adult hockey carries an inherent risk of injury due
to its physical nature.
 The player and parent/guardian agree to follow all club rules, codes of conduct,
safeguarding policies, and any instructions issued by coaches or match officials.
